In order to obtain a license in the state of Utah for a medical cannabis pharmacy, the applicant must provide a Utah Cannabis Pharmacy bond in the amount of $100,000. The bond guarantees the
strict compliance with all applicable provisions of, and orders, rules and regulations issued pursuant to, Utah Code Title 26, Chapter 61a and Utah Administrative Code R380-400 through R380-411 by the bond principal. Applicants under the same ownership may submit license applications for multiple locations in the same region or in multiple regions but each location requires its own application and payment of a separate $2,500 application fee and a separate Utah cannabis pharmacy bond. - Medical Cannabis Home Delivery Pharmacy License Fee – Urban (Annual Fee)* $69,500
- Medical Cannabis Home Delivery Pharmacy License Fee – Rural (Annual Fee)* $52,500
*Licensees approved by the Department as a home delivery pharmacy must provide in-person service at
the facility AND they must provide home delivery service to cardholders. Payment of the home delivery
pharmacy licensing fee amount allows a pharmacy to provide in-person AND home delivery service.
