The Oklahoma Health Spa Registration Bond is required by the Oklahoma Department of Consumer Credit who would like to establish a health spa in Oklahoma. A health spa is any person, firm, corporation, organization, club or association engaged in a program of physical exercise, which includes the use of one or more of a sauna, whirlpool, weight-lifting room, massage, steam room, or exercising machine or devise, or exercise rooms, or engages in the sale of the right or privilege to use exercise equipment or facilities.
The bond protects the State and the bond holder’s clients by ensuring that the bondholder, or principal, will fully observe and abide by the regulations set forth in Title 59, sections 2000 through 2012, and any other legislature relevant to the bond holder’s business.
In addition to the $5,000 surety bond, applicants must also submit the following:
- Payment of $200 for the one-time investigation fee and $30 for the registration fee
- Completed application form
- A letter stating where records will be kept in Oklahoma
- Presale information
- A copy of all the basic documents establishing the entity
