- A certificate of worker’s compensation insurance
- The names and pertinent information of all persons with interest in the farm labor contractor operation of the applicant.
- Proof of financial responsibility in the form of a surety bond issued by a surety company licensed to operate in Idaho. The surety bond is set at $10,000 if the farm labor contractor has no more than 20 employees and $30,000 for more than em employees.
