  3. Scanned copy of Local License
  4. Scanned copy of Warehouse Lease/Deed
  5. A Notarized Georgia Alcohol & Tobacco Personnel Statement
  6. A notarized Financial Affidavit
  7. Georgia Tobacco Distributors Tax Stamp Bond
  8. Cigar & Cigarette Distributor License Performance & Liability Bond. The bond amount for original applications is $5,000.
  9. Power of Attorney
  10. Citizenship Affidavit along with secure and verifiable documents, i.e. government-issued photo ID (requires notarization)

Tobacco Wholesaler/Distributor (Out-of-state) Requirements

  1. Scanned copy of the current annual registration from the GA Secretary of State
  2. Scanned copy of a Letter of Good Standing
  3. Scanned copy of a List of Products to be sold
  4. A notarized ATT-17 Georgia Alcohol & Tobacco Personnel Statement
  5. Georgia Tobacco Distributors Tax Stamp Bond
  6. Cigar & Cigarette Distributors License Performance & Liability Bond. The bond amount for original applications is $5,000 and is available from Surety1.
  7. Power of Attorney (if applicable)
  8. Citizenship Affidavit along with secure and verifiable documents
