2. ATT-17 Georgia Alcohol & Tobacco Personnel Statement (requires notarization)
3. Citizenship Affidavit along with secure and verifiable documents, ie ID
4. Financial Affidavit (requires notarization)
5. Scanned copy of Federal Basic Permit
6. Scanned copy of Local License
7. Scanned copy of the Warehouse Lease/Deed
8. Scanned copy of current annual registration from the GA Secretary of State’s Office
9. Scanned copy of the Warehouse Lease/Deed
10. Power of Attorney (if applicable)
11. Fees ($500)

The bond has a mandatory expiration date of 12/31 each year. The bond states that the bond holder will promptly pay all sums which may be required due to taxes, license fees, rental charges, including all penalties and interest.
