What are the requirements to get a Home Food Service Plan License in Colorado?

Sellers of HFSP can be individuals or companies that sell meat products or frozen meat at the home of a consumer. In order to obtain the license required to be a seller of HFSP, the applicant must submit the following:

1. A completed Home Food Service Plan Seller License application form
2. A completed Citizenship Verification Form
3. Application fee – this differs depending on the number of salesmen in your company:

*sole operator – $350

*2-5 salesmen – $750

*6-10 salesmen – $1250

*11-15 salesmen – $1500

*16-20 salesmen – $2000

*21+ salesmen – $3500
