  • One set of fingerprints for each principal of the business
  • Fingerprint background waiver
  • Behind the Wheel activity: Insurance certificate showing automobile liability coverage with the department as a certificate holder.
    • Bodily injury or death for one person in any one accident $100,000
    • Bodily injury or death for two or more persons in any one accident $300,000
    • Property of others in any one accident $50,000 (CSL = combined single limit is acceptable)
  • Copy of City or County business license

  • Federal Employer Identification Number
