What are the requirements in order to operate a Florida Assisted Living Facility?

To expedite the licensing process, make sure to have the following requirements ready:

  • You must have the information of the provider, owner, contact person, property owner, all the personnel, disclosures, provider fines and financial information. All these will be put on the application form which must be completed and submitted to the Florida Agency for Health Care Administration.

  • Aside from those mentioned above, you must also supply information on the bed counts, the services offered, and some supporting documents such as a current general liability insurance coverage and fire safety inspection report.


  • For assisted living facilities which will serve as a power of attorney for residents or a representative payee for social security, veteran’s or railroad benefits, a Florida Assisted Living Facility Surety Bond must be filed along with the application form. This bond should be in an amount to twice the average monthly aggregate income or personal funds due to residents. This bond ensures that the ASL facility will handle said funds properly and should there be any violation of the agreement, the resident will be properly compensated.
