The bond states that the bondholder shall faithfully perform all agreements and contracts entered into with students as disclosed by the application for a license and comply with all provisions of governing regulations. There is no set bond amount, but it should not be less than $10,000.

• Description and statement of credentialing requirements for faculty
• List and address of facility(ies) used for educational instruction
• Facility safety plan for each site
• Inspection reports for each site
